Fall 2009 Registration Sessions:
For Travel Registration, please bring completed NJYS
Player Membership, MCYSA SAGE and Medical Release
forms, along with passport size picture and copy of
birth certificate/passport. The medical release form must be
notarized. All forms are located on the
Forms page.
For
Clinic Registration, please complete appropriate
Kindergarten, 1st or 2nd grade form and notarized
medical release form.
